Currently seeking a Consultant Pharmacist in Long Term Care – Full-time remote position. Must have NJ Board of Pharmacy license.

This position pays $48.00 - $50.00 per hour.

Responsibilities A day in the life of a Remote Consultant Pharmacist:
- Perform medication regimen reviews for new admissions/readmissions to Long Term Care facilities using clinical skills based on patient's health history to evaluate appropriateness, allergy information, drug interactions, medication errors, transcription errors, medication reconciliation, duplicate therapy, polypharmacy, proper dosing and administration, safety, benefits, risks, and cost-effectiveness of medication therapy
- Evaluate patients’ medications for a change in their status (condition)
- Evaluate Antimicrobial therapy for Antibiotic Stewardship

Extra Credit:
- Prior clinical experience in long term care
- Active CCP
Requirements Authorized to work in the US without sponsorship
Current NJ Board of Pharmacy License
Minimum Bachelors Degree in Pharmacy
