Job Description

The Supervisor, Intensive Community Care, RN is a multi-market leadership position responsible for leading a team of Intensive Community Managers (ICCMs) in specific markets to deliver on managing their case load by providing onsite and remote planning, clinical group leadership and expert technical guidance to their team. Directly reporting to the Manager of ICC, this Supervisor is responsible for their teams members assigned patient case load whose role is to proactively manage their assigned patients to prevent unnecessary hospital arrivals. The success of this role will be measured by number of hospital arrivals for the patients in their care. The supervisor’s role works closely with the manager to achieve success in assigned markets and provides manager coverage when needed for the region. The Supervisor of Intensive Community Care, RN, Complex Care will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures. The Supervisor of Intensive Community Care (ICC) directly manages specific market teams that will include multiple areas and ICCMs. As a member of the Complex Care Team, this role is responsible for high-value care across multiple regions, to serve as coach, mentor/trainer to all members of care management team, giving guidance in best practices, troubleshooting to optimize the value of care in hospital and community setting. Essential duties include supervising ICCMs to reduce hospital arrivals, managing employment cycle of direct reports, providing coverage for the Manager when needed, advocating for patients' optimal health and resource utilization, consulting with providers for patient referrals, mentoring staff, facilitating medication reconciliation and education, assessing patient progress, educating ICCMs on care plans, assisting with policy development, attending relevant meetings, conducting educational programs, identifying best practices, improving workflow efficiency, developing SOPs, completing individual care plans, monitoring clinical and financial coordination of treatment plans, facilitating interdisciplinary rounds, monitoring progress towards goals and KPIs, serving as preceptor, monitoring quality of outpatient services, and performing other duties as assigned. The role requires proven case management experience, knowledge of healthcare management and reimbursement, effective communication, organizational skills, problem solving, compassion, and ability to travel up to 20%. Education and experience criteria include an Associate degree in Nursing (required), Bachelor’s Degree in Nursing or related field preferred, Registered Nurse with over 3 years clinical experience, compact license if available, minimum 2 years onsite case management at ChenMed or related hospital experience, current nursing licensure, BLS certification within 90 days, valid driver’s license, and preferred certifications in Case Management.
