\tPatient Population: Young adults to geriatrics
\tReporting: Works under direction of RN, physician, Site Supervisor, or Site Manager
\tProvide patient care within the scope of the Vocational Nurse Practice Act
\tAssist with assessment, planning, implementation, and evaluation of patient care
\tImplement and document individualized discharge plans
\tCollect, handle, and process specimens appropriately
\tPerform complete and accurate patient charting
\tMaintain accurate equipment and quality measure records
\tAssist with care and maintenance of department equipment
\tCollaborate with interdisciplinary care teams across the continuum
\tEnsure a safe, ethical, and HIPAA-compliant care environment
\tRecognize ethical, legal, and medical dilemmas and escalate appropriately
\tParticipate in continuous quality improvement initiatives
\tMinimum 1 year of Home Health experience REQUIRED
\tAt least 1 year of experience in a healthcare facility
\tSpecialty clinics may consider new grad LVNs (not applicable for this Home Health role)
\tActive California LVN License
\tBLS Certification (current)
\tPhlebotomy Certification
\tBasic keyboarding skills ( minimum 35 WPM )
\tProficiency with Microsoft Office (Outlook, Word, Excel)
\tStrong communication and interpersonal skills
\tCustomer-focused mindset
\tStrong attention to detail and organizational skills
\tAbility to prioritize tasks and meet deadlines
\tAbility to work effectively in a multidisciplinary team
\tKnowledge of infection control, safety measures, and proper body mechanics
\tAbility to follow instructions and adapt to patient age and condition
\tExperience with Electronic Medical Records (EMR)
\tKnowledge of medical terminology
\tKnowledge of patient scheduling, registration, and insurance verification
\tOrder entry systems experience
\tDemonstrates cultural competence and sensitivity
\tAdapts plan of care to meet multicultural patient and family needs
\tMaintains professionalism when interfacing with:
\tPhysicians
\t\tHealthcare staff
\t\tInsurance companies
\t\tPatients and family members
\t\tStrong troubleshooting and problem-solving skills
\tHome Health setting
\tOutpatient-based support structure
\tMultidisciplinary collaboration
\tHigh School Diploma or equivalent
\tHome Health experience is mandatory
\tMust hold active CA LVN license BLS
\tStrong documentation and communication skills required
